LPPD DISCLOSURE STATEMENT

1. The owners of personal data identified as the data subject (“Applicant”) in the Law on The Protection of Personal Data No. 6698 (“Law”), have been granted the right to submit a number of requests to the Controllers for the processing of their personal data in Article 11 of the law. You have the following rights under the applicable data protection legislation;

• to learn whether or not the personal data relating to you are processed,
• if processed, to request information with regard to processing,
• to learn purposes of the processing and whether they are used for such purpose or not,
• to request the restriction of processing under certain circumstances,
• to know the third persons within or outside the country to whom the personal data are transferred,
• to request correction and update of the personal data if the data is processed incompletely or inaccurately,
• to request deletion or destruction of the personal data when purposes of processing cease to exist,
• to request notifying third parties to whom the personal data are transferred, about deletion, destruction and correction,
• to object to negative consequences about you that are concluded as a result of analysis of the processed personal data by solely automatic means, and
• to claim compensation for the damage arising from the unlawful processing of your personal data.

1. In accordance with the first paragraph of Article 13 of the law, applications to be made for these rights must be submitted in writing to.
2. In this context, the applications to be made to TØMPSEN as “written”, will be printed out of this form;

• By submitting the signed copy of original by hand or by mailing it to’’Maslak Mah. Sanatkarlar Sk. Eclipse Maslak No: 2 E İç Kapı No: 9 Sarıyer/ İstanbul’’ address through notary public or;

1. If another application method is determined and an announcement made by the board, announcements regarding these methods will be made via TØMPSEN’s website.
2. Your applications submitted to TØMPSEN will be answered within 30 (thirty) days from the date of your request to TØMPSEN according to the nature of the request, in accordance with Article 13 paragraph 2 of the law. TØMPSEN’s responses will be sent to you in writing or via electronic media in accordance with Article 13 of the law.
3. If your application requires an additional cost, you will be required to pay the fee determined by the notification about the application procedures and principles to the Controller issued by the Personal Data Protection Board. If your application is answered in writing, there will be no charge until the first 10 (ten) pages, 1 TL processing fee will be charged for each page above 10 (ten) pages. If the answer to your application is given in a recording medium such as a CD or flash memory, the cost of the recording medium will be reflected to you.
4. TØMPSEN reserves the right to make changes to the decision of the board regarding the application procedures or to any future legal arrangements.

This form has been prepared in order to determine your relationship with TØMPSEN and to determine the full extent of your personal data, if any, processed by TØMPSEN, so that your relevant application can be answered correctly and within the legal period. Additional documents and information (ID card, passport or driver’s license copy, etc.) reserved the right to demand by TØMPSEN for identification and authorization purposes to eliminate legal risks that may arise from unlawful and unfair data sharing and in particular to ensure the security of your personal data. In the event that the information you submit under the Form is not accurate or up to date, or an unauthorized application is made, TØMPSEN assumes no liability for such false information or requests arising from the unauthorized application.
