TØMPSEN Customer’s Explicit Consent Form

I do hereby agree, acknowledge and declare that TØMPSEN has fulfilled its obligation to inform me pursuant to the relevant regulations, communiqués, deeds of undertaking and principles and rules as provided for in the Decisions issued by the Personal Data Protection Board and in particular in accordance with the Law No 6698 on the Protection of Personal Data (hereinafter collectively be referred to as the Legislation on the Protection of Personal Data (“PDP Legislation”) in an accurate, full and detailed fashion. I, being fully informed beyond any doubt about my rights regarding processing of my personal data, do hereby explicitly authorize and give my free and voluntary consent to the Data Controller (TØMPSEN) and its directors and officers, natural persons or legal entities of private law, cargo companies, courier companies and suppliers for the collection and processing of my personal data that fall within the scope of the customer disclosure statement form including my communication details, my personal identifiable information and in particular my other personal data provided by me to TØMPSEN within the framework of a contract(s) drawn up and concluded between TØMPSEN and me, as a customer, including without limitation for collection, recording, structuring, storage, adaptation or alteration, consultation, use disclosure by transmission, dissemination, making available for collection or use, alignment or combination, categorization, disclosure, sharing and transfer of it either wholly or partly, whether through automatic means, or through non-automatic means or other means and transfer of my personal data and processing in similar situations for a period of time not exceeding the achievement of the purposes for which they are collected and processed and in compliance with the mandatory time limits as prescribed by law and strictly complying with all applicable rules and principles regarding processing of such personal data and taking and implementing all necessary measures to safeguard and maintain the privacy and confidentiality of it.
